Headline 1 and Description 1 almost always show; Headline 2, Headline 3, and Description 2 appear based on predicted performance, so never write a headline that only makes sense as the second half of a sentence. Pinning to Headline 3 or Description 2 does not guarantee the asset runs, which surprises a lot of advertisers who pin a legal disclaimer there and assume it is covered.

Ad Strength measures variety, and pinning is usually what kills it

Google grades every responsive search ad from Poor to Excellent, and the score is mostly a completeness and variety check: did you supply enough headlines, are they distinct from each other, do the descriptions add information rather than repeat, and are your keywords represented. It is not a quality judgment on your writing and it is not a ranking factor you can bank on, but a Poor rating is a genuine signal that the system has too few combinations to test with. The single most common cause of a low score is pinning. Advertisers pin a brand name to Headline 1 and a compliance line to Description 1 for perfectly good reasons, and in doing so they collapse the number of combinations Google can build, which drops Ad Strength and narrows the ad. If you must pin, pin one slot rather than three, and consider pinning two or three different assets to the same position so the system still has a choice. The wider fix is supplying enough unpinned material elsewhere that the constrained slot is not the whole ad.

Fifteen distinct headlines is a writing problem, not a counting problem
